Floyd and Gordon County Gibson, Townley, Lathan, Harkins, Vaughn Demonstration Prevention Home Visiting Program in Floyd and Gordon counties to provide parental support and promote health-development of kids. DFCS, PH and community partnership with primary/secondary prevention.

Funded with $125,000 in DFACS and State money
District 1-1 Team Naves, Robinson, Terry, Wetherington The Women's Health Model will provide a "single point of entry" for all Women's Health services from puberty to the end of life. Model in Chatooga and Gordon counties.

Dade County Team DeMarche, Page, Bradford, Bradford Three part initiative: the construction of a wellness center; the construction of walking/fitness trails; and the development and implementation of a preventive health education program in grades 1-8
Whitfield County HD Woodard, Babb, Christie, Kennedy International Inclusion Center: Provide health-related services to new immigrants (primarily Hispanic) and foster transition into the community through the schools.

Hertford-Gates Health Agency Harrell, Horne, McLawhorn, Pierce, Wiggins Child care for the employees of Hertford-Gates Health Agency and other local government agencies.  The childcare center will have a strong emphasis on guidelines for healthy eating, daily routine exercise, and a structured learning environment for 15-25.

Appalachia I Health District Craig, Trado, Little, Ellenburg Osteoporosis awareness and prevention program aimed at improving the identification, screening and referral of those at risk.
DHEC White, Weidenburner, Morales, Williams, Hardee One stop health complex project located at the former Chopee High School, seeks to provide clinical and other services to underserved African American community.

Bureau of Drug Control Baker, Burgess, Kotecha, Busbee, Smith Program to control illicit release and misuse of narcotics therapy to reduce the overdose and long-term arrest rates within the community
Lord Fairfax Arnold, Chrisman, Dowden, Guthrie, Lewis-Snow, Snarr Healthy Teens - A private, non-profit community-based program that identifies youth at risk for poor school attendance, poor grades, and poor conduct.  Private case management funded by the community.

Peninsula Keene, Wright, Holden, Sher, Gill Food safety "Outbreak" game aims to increase awareness of food safety issues on a college campus and with the 18-24 y.o. age groups
VDH Medical Examiners Office Morrison, Reynolds, Taylor, Little-Bowser Virginia Dept of Vital Records - a tutorial based computer CE program on completion of the death certificate for physicians.  Partners are the Virginia Medical Society, Virginia Medical Universities and the Virginia licensing board.
VDH Ellison, Stover, Leary Use of distance education technology and video conferencing to replace on-site public health training programs, thereby making traninig programs more cost effective and feasible to reach a wider public health audience.
Portmouth HD Brown, Tsegaw In partnership with area Agencies of Aging, Hospitals, Physicians, Pharmacists and Pharmaceutical Companies,  produce and distribute an educational documentary video on Medication Management among adults ages 60 and older.
